POPULATION SERVED
At risk older and disabled adults, low-income, high risk for social isolation, none or very little regular support and are at a high financial risk with very limited resources.
OUTCOME DETAILS
“The Daily Call Sheet is proving to be very successful”: 67 recipients and expanding weekly First phase only – We will conduct the survey again at the twelve months and two year points to measure if participants have moved up on the scale.
SOCIAL ISOLATION OR CONNECTION MEASURES USED
Intake Survey incorporates the Evidence based UCLA Loneliness Scale and several questions form the Duke UNC Functional Social Support questionnaire to measure social isolation
